You need not provide information about felony convictions which are not related to the job for which you are applying. A conviction will not necessarily constitute disqualification, and its nature, age and job-relatedness will be considered.
I understand that this application and any other employment-related documents which may be distributed from time to time
do not constitute a contract of employment and that I or the company may terminate the employment relationship at any
time, for any reason, with or without notice at the option of either myself or PGE. No promises regarding employment have
been made to me, and I understand that no such promise or guarantee is binding upon PGE unless made in writing.
I understand that upon request I may also be asked to allow PGE to request a consumer report under the guidelines set
forth by the FCRA (Fair Credit Reporting Act).
In making this application for employment, I understand that an independent investigation may be made whereby job- related information will be obtained regarding my scholastic, employment, and criminal records. I understand that any employment is conditioned on a background check. Any information obtained will be used to determine work assignment or employment eligibility under the company’s employment policies. I hereby authorize the company to investigate any and all matters contained in this application. I authorize and consent for full release of records (oral or written) to authorized representatives of the company. I also authorize my previous employer(s) to release to the company, and the company to receive, any and all information concerning my past work history, including disciplinary records, to be used in connection with consideration of my application for employment. I also waive all statutory rights under any Local, State or Federal law and any private causes of action whatsoever which might otherwise be available to protest the release, receipt and/or consideration of such information including but not limited to any right to notification of release by my previous employer.
If I am offered employment I agree to submit to a medical examination and drug test before starting work. If employed, I also agree to submit to a medical examination or drug test at any time deemed appropriate by PGE and as permitted by law. I consent to such examinations and tests, and I request that the examining doctor disclose to PGE the results, which shall remain confidential and segregated from my personnel file. I understand that my employment or continued employment will be that I abide by PGE’s Drug and Alcohol Policy and that preference is given to non-tobacco users.
I understand that filling out this form does not indicate there is a position open and does not obligate PGE to hire. If hired, I agree to abide by all PGE work rules, policies and procedures. PGE retains the right to revise its policies and procedures, in whole or in part, at any time
